Who is Premise Health?
How do I make an appointment?
You can call a center directly or stop by during regular business hours and make an appointment in person. You can also visit mypremisehealth.com or schedule an appointment through the My Premise Health app. If you’re unable to keep your scheduled appointment, please call to cancel and/or reschedule.

Will my current medical provider have access to my health records if I receive care at one of your centers?
After you have been treated at our health centers, you may request to send records to another medical provider. If your community provider uses Epic (or MyChart) for their electronic health records (EHR), your records will be available via Epic’s Care Everywhere platform and vice versa.
How much does a visit cost?
$15 copay or $30 access fee depending on which of the employer sponsored insurance plans were chosen, or if the ESI was waived.
Who is eligible to use the Notre Dame Wellness Center and Pharmacy?
Services are available to benefit-eligible faculty and staff and benefit-eligible dependents.
What is My Premise Health?
My Premise Health is a secure online and mobile tool that puts your health information in your hands. It connects you with your healthcare experience by providing convenient access to schedule and manage appointments, message your providers, conduct virtual visits, view your lab results, request medication refills and more.
Is my health information secure? Does Notre Dame have access to my personal health information?
Your personal health information is confidential, and your data is kept secure. Premise Health operates in accordance with HIPAA and works diligently to protect all health records. We can’t share any personal health information with your employer or anyone else without your permission unless it pertains to a work-related health injury. Premise Health also operates through a private network and software systems that cannot be accessed by other members or company staff. You can review a notice of privacy practices within the Notre Dame Wellness Center & Pharmacy.
How do I contact the pharmacy?
You can call the pharmacy at (574) 271-5622 to speak with a member of the pharmacy team. If you are experiencing a medical emergency, please dial 911.
How does my provider send my prescription to the Notre Dame Wellness Center Pharmacy?
You can easily drop off your prescriptions at the Notre Dame Wellness Center Pharmacy or your provider can e-prescribe them directly to the pharmacy. The providers and pharmacy at your wellness center act as one care team. Your provider will send your prescription to the pharmacist after your appointment so you can pick it up the same day, making for a seamless experience.
